A Lesson in Heart Health: Cholesterol

Few topics in health are as debated as cholesterol — so rather than pick a side, let's lay out what's settled and what's genuinely contested.
What cholesterol actually is
First, a reframe: cholesterol isn't a villain, it's essential. Your body uses it to build cell membranes, make hormones and produce vitamin D. It travels in the blood packaged in lipoproteins, most familiarly LDL ("low-density") and HDL ("high-density"). LDL is popularly cast as "bad" and HDL as "good", though the real biology is more subtle than those labels suggest. So the question was never whether cholesterol is bad — it's essential — but what role blood cholesterol levels play in heart disease.
The mainstream view
The dominant position in cardiology is clear and strongly evidenced.
A consensus statement from the European Atherosclerosis Society, drawing on genetic, epidemiological and clinical trial evidence, concluded that LDL cholesterol has a causal role in atherosclerotic cardiovascular disease (Ference et al., 2017).
The strongest strands of that case are genetic (people born with lifelong high LDL have high heart-disease risk, and those with lifelong low LDL have low risk) and interventional (lowering LDL reduces cardiovascular events). On this view, LDL is a primary driver of arterial plaque.
How cholesterol and heart disease became connected
The mechanism behind that consensus is worth spelling out, because it explains why both sides of this argument can be partly right. Atherosclerosis begins in the artery wall rather than in the bloodstream. LDL particles pass from the blood into the wall of the vessel, and some are retained there. Once retained, they are chemically modified — oxidised, among other changes — and the immune system responds to the modified particles as it would to any foreign material. White blood cells move in, take up the altered lipids and become the foam cells that form the fatty streak, and the resulting inflammatory response drives the plaque's growth and eventual instability.
Read carefully, that account contains both claims at once. The process needs LDL to be there, which is the mainstream point. It also runs on oxidation and inflammation, which is the sceptics' point. They are not rival explanations so much as different stages of the same one, which is why the sensible reading is rarely "cholesterol or inflammation" and usually "cholesterol and inflammation".
The sceptical view
Yet a vocal minority of researchers — and many in the natural-health community — push back, and their arguments deserve a fair hearing. They point out that total cholesterol is a weak predictor of heart disease in many populations; that some people with high cholesterol never develop heart disease while some with low cholesterol do; and that focusing narrowly on LDL may distract from other important drivers such as chronic inflammation, insulin resistance, and the role of refined carbohydrates and sugar. Some argue the type and size of LDL particles matter more than the raw number, and that dietary cholesterol has far less effect on blood cholesterol than once believed. A reasonable way to hold it
Where does that leave a thoughtful reader? Probably here: LDL almost certainly plays a real causal role, especially at the extremes — but heart disease is multifactorial, and inflammation, metabolic health, blood pressure and lifestyle all matter alongside cholesterol numbers. That's an empowering conclusion, because so many of those factors respond to how you eat, move and live, as we cover in how to reduce inflammation naturally. What a cholesterol test actually reports
Part of the confusion here comes from the test itself. A standard lipid panel reports total cholesterol, HDL cholesterol, triglycerides and an LDL figure that, in many labs, is calculated from the others rather than measured directly. Non-HDL cholesterol — total minus HDL — is often reported too, and captures the cholesterol carried by all the potentially plaque-forming particles rather than LDL alone.
The subtler point is what LDL cholesterol measures. It is the quantity of cholesterol being carried by LDL particles, not the number of particles doing the carrying, and those two can diverge: someone can carry a lot of relatively cholesterol-poor particles and show an unremarkable LDL number. This is the legitimate core of the sceptics' argument about particle size and quality, and mainstream lipidology takes it seriously — which is why measures of particle count, such as apolipoprotein B, are increasingly discussed. Practically, this means a single number on a printout carries less information than it appears to, in either direction.
Why risk is never one number
Clinicians do not, in practice, read a cholesterol result on its own. Cardiovascular risk is assessed from the whole person — age, sex, blood pressure, smoking status, diabetes, kidney function, family history and the cholesterol figures together — because the same LDL value carries very different weight in a 35-year-old non-smoker and a 65-year-old with high blood pressure and diabetes. That is why two people with identical results can reasonably be given different advice, and why an isolated number found online means little without the rest of the picture.
It also explains where the debate genuinely narrows. At the extremes, few people argue. Familial hypercholesterolaemia — an inherited condition causing very high LDL from birth — carries a well-recognised risk that no one on either side dismisses, and it is worth knowing about if heart disease appeared early in your family. The real disagreement is about the large middle ground of moderately raised readings in people who are otherwise well, and that is exactly where individual risk assessment does the work.
Where both camps agree
Strip out the argument and a substantial common ground remains, which is the most useful part of this for most readers. Not smoking, regular movement, a diet built largely on whole foods, healthy blood pressure, sleep and moderate drinking are endorsed by everyone in this debate. They act on inflammation, insulin resistance, blood pressure and lipids at the same time, so you do not need to settle the causal question to act sensibly.
What none of this supports is using scepticism as a reason to skip testing, ignore a strikingly high result, or stop a treatment already prescribed. A live scientific debate about how much a risk factor contributes is not evidence that it contributes nothing, and any decision about medication belongs with you and your doctor, with your full risk picture in front of you.

No. Researchers disagreeing about how much a risk factor contributes is not the same as it contributing nothing, and a very high reading is worth taking seriously and discussing with your doctor.
