Inflammation has become a health buzzword — usually cast as the villain. The truth is more interesting: without it, you couldn't heal at all.

Healing versus harmful: understanding inflammation

These days almost every ailment is linked, directly or indirectly, to inflammation. But is inflammation really the enemy? Not quite — because it's also a cornerstone of the body's natural healing, and no injury or infection heals without it. You already know it well: the pain, swelling and heat around a cut finger or a sore throat is inflammation at work.

What is inflammation?

Inflammation is the immune system's response to a threat — whether an external one such as a virus or bacterial attack, or an internal stress such as a toxin or oxidative damage. Understanding what is inflammation really comes down to recognising there are two very different kinds: acute and chronic.

Acute vs chronic inflammation

The difference between acute vs chronic inflammation is essentially the difference between a fire brigade that arrives, does its job and leaves, and one that never stands down.

Acute inflammation: the body's rapid repair response

When you face a microbial attack or physical trauma — a burn, a cut, surgery, a sprain — the immune system floods the site with specialised cells and chemicals. This is acute inflammation: the short-term response that protects, repairs and heals. Roughly, it unfolds like this:

  1. Blood vessels widen, increasing blood flow to the injured area.
  2. Capillaries become more permeable, letting white blood cells, fluid, proteins and nutrients move into the tissue.
  3. White blood cells — chiefly neutrophils and macrophages — arrive first, destroying invaders and clearing away dead and damaged tissue.
  4. Immune cells release cytokines, a large family of signalling proteins (including interleukins, interferons and tumour necrosis factor) that coordinate the response, rally other cells and help stop an infection spreading.
  5. The process resolves in healing and repair — or, if it fails to switch off, tips into chronic inflammation.

The familiar signs of inflammation in body tissues — swelling, redness, heat, pain, sometimes reduced movement — are simply evidence that this system is doing its job. They're uncomfortable, but short-lived, and a sign the body is fighting back.

Chronic inflammation: when the fire won't go out

Ideally, once the threat is gone, the inflammatory response shuts down. Problems begin when it doesn't. Sometimes the body can't fully clear the original trigger; sometimes repeated bouts of acute inflammation train the immune system into an abnormal, always-on state. Much of modern life nudges it that way.

Common drivers of chronic, low-grade inflammation include a diet heavy in sugar, processed food and saturated fat; lack of exercise; ongoing stress and poor sleep; persistent infections; environmental toxins and pollutants; misdirected immune responses as in rheumatoid arthritis; smoking; and heavy alcohol use.

When the immune system stays on high alert, it keeps sending cells and inflammatory proteins to the same sites, relaunching the response again and again. Sometimes it even attacks the body's own tissues — the basis of autoimmune conditions such as lupus and rheumatoid arthritis. This slow, smouldering, systemic inflammation is now regarded as a root contributor to a long list of conditions.

Systemic chronic inflammation, driven by factors such as poor diet, physical inactivity, chronic infection, environmental toxins and psychological stress, contributes to leading causes of illness including cardiovascular disease, diabetes, cancer and neurodegenerative disorders (Furman et al., 2019).

Can you actually measure inflammation?

Given how often chronic inflammation is invoked, it is reasonable to ask whether it can be measured. Partly, yes — but with real limitations worth understanding before paying for a test.

The marker used most often is C-reactive protein, a substance the liver makes when inflammation is present. The standard version is used clinically to detect the large rises that come with infection or a flare of an inflammatory condition. A more sensitive version, high-sensitivity CRP, can pick up the much smaller elevations associated with low-grade inflammation, and is sometimes used alongside cholesterol when assessing cardiovascular risk. The erythrocyte sedimentation rate, or ESR, is an older test reflecting a similar picture more slowly.

The limitation is that these markers are non-specific. A raised CRP tells you inflammation is happening somewhere; it does not tell you where, why, or whether it matters. It rises with a cold, after strenuous exercise, after an injury, in obesity, and across a long list of conditions. A single reading in isolation is easy to over-interpret in either direction, which is why doctors read it alongside symptoms and other results rather than on its own.

It is worth knowing, too, that the things which shift these markers are the unglamorous ones already listed above: weight, activity, smoking status, sleep and the quality of the diet. There is no shortcut that lowers a number without changing the circumstances producing it, and a marker nudged down while the drivers remain untouched has not achieved a great deal.

This is part of why the commercial inflammation panels sold direct to consumers deserve some scepticism. Measuring more markers does not solve the underlying problem that none of them is specific, and a number without clinical context tends to generate anxiety rather than answers. If you have symptoms that concern you, the more useful route is a conversation with your doctor, who can decide which tests would actually change what happens next.

So, is inflammation good or bad?

Neither label quite fits. Inflammation plays a genuinely complex role — some of it protective, some of it damaging — and even cytokines can be pro- or anti-inflammatory depending on context. It's too simple to brand acute inflammation "good" and chronic "bad". In both cases the body is trying to protect you against a perceived threat. The real skill is reading what your body is signalling and addressing the underlying drivers, rather than simply silencing the symptoms.
