Why is Sugar So Bad?

Sugar is delicious, cheap and everywhere — and mounting research suggests that combination is doing us far more harm than a few extra calories.

Not all sugar is a problem: the sugars naturally present in whole fruit, grains and vegetables come packaged with fibre and nutrients. The trouble is added sugar — the kind stirred into processed foods and drinks, delivering calories with little else. Once you look past weight gain and diabetes risk, the negative effects of sugar reach surprisingly far. Here's how sugar affects the body once the added kind starts to mount up, system by system.

Insulin resistance and type 2 diabetes

Insulin lets your cells take glucose out of the blood. Flood the system with sugar repeatedly and the pancreas pumps out more and more insulin — until cells stop responding well to it. That's insulin resistance, which sets the stage for metabolic syndrome, pre-diabetes and eventually type 2 diabetes.

Weight gain and appetite

Sugar drives weight gain in more ways than one. Insulin spikes interfere with leptin, the hormone that signals fullness — so you can feel hungry even after eating, and crave more. This is one of the clearer sugar health risks, because the weight gain then feeds back into insulin resistance and diabetes risk.

Liver damage

Added sugar is high in fructose, which is processed largely by the liver. In excess, that fructose is converted to fat, and fat building up around the liver can lead to non-alcoholic fatty liver disease. Researchers have noted that the pathways handling fructose overlap with those handling alcohol, and that heavy fructose intake can drive similar problems — and studies suggest fructose-related liver stress can occur even without obesity or excess total calories.

Heart disease

A sugar-heavy diet is a reliable way to raise cardiovascular risk. Sugar promotes arterial inflammation and plaque, worsens the cholesterol picture (raising triglycerides and LDL, lowering HDL), and — through insulin resistance and weight gain — compounds the risk further.

In a large prospective study of US adults, higher intake of added sugar was associated with a significantly increased risk of dying from cardiovascular disease (Yang et al., 2014).

Brain health and dementia

The brain isn't spared. Insulin resistance reduces insulin signalling in the brain and can impair the delivery of glucose — the brain's main fuel — which is why some researchers have nicknamed Alzheimer's "type 3 diabetes" (a label that reflects a hypothesis, not a settled diagnosis). High blood sugar has also been linked with dementia risk more broadly.

In a study of older adults, higher average glucose levels were associated with an increased risk of dementia — even in people who did not have diabetes (Crane et al., 2013).

Diets high in added sugar have also been linked, largely in animal research, with lower levels of brain-derived neurotrophic factor (BDNF), a protein important for memory and learning — hinting at a vicious cycle between poor glucose control and brain health.

The sugar crash

That post-cookie energy dip is real. A big sugar hit triggers a rapid insulin surge, which can send blood sugar dropping too low too fast — bringing on fatigue, irritability, poor concentration and, of course, more sugar cravings. It can take as little as half an hour to swing from high to crash.

Cancer and skin

The links here are more preliminary but worth noting. Sugar drives inflammation, which is broadly implicated in cancer development, and laboratory research has explored how cancer cells metabolise fructose; insulin resistance and its free-radical damage are also areas of study. Separately, excess blood sugar reacts with proteins to form advanced glycation end products (AGEs), which damage the collagen and elastin that keep skin firm — contributing to wrinkles and sagging over time.

Where added sugar actually hides

Cutting added sugar is harder than it sounds, mostly because a good deal of it is not in the foods people think of as sweet. Sauces, dressings, marinades, breads, breakfast cereals, flavoured yoghurts, ready meals, soups and so-called healthy snack bars are all common sources, and in many of them the sugar is doing a technical job — balancing acidity, improving texture, extending shelf life — rather than making the product taste like a dessert.

Labels do not always make this obvious. Sugar appears under a long list of names: sucrose, glucose, fructose, dextrose, maltose, glucose-fructose syrup, high-fructose corn syrup, invert sugar, molasses, honey, agave and fruit juice concentrate among them. Because ingredients are listed by weight, spreading the sweetening across several of these pushes each one further down the list even when the total is substantial. The clearer signal is usually the nutrition panel rather than the ingredients list: look for the sugars figure, and where the label separates them, the added sugar figure in particular.

Drinks deserve their own mention. Liquid sugar arrives quickly, comes without fibre, and does very little to satisfy hunger, so a sugary drink tends to be added to what you eat rather than displacing any of it. For most people wanting to reduce added sugar, drinks are the single highest-yield place to start.

Cutting back without white-knuckling it

Going cold turkey works for some people and rebounds for others. The gentler route is to lower the background level rather than attempt perfection: change the sweetened drink first, then the everyday breakfast, then the routine snack, and leave genuine treats alone. The palate adjusts, and foods that tasted normal a month ago start to taste cloying — which does more for long-term intake than willpower ever does.

It also helps to eat sugar in company rather than alone. Something sweet eaten alongside protein, fat or fibre produces a gentler rise and fall than the same thing on an empty stomach, which blunts both the crash and the craving that follows it.

How much added sugar is too much?

Guidelines vary, but major health bodies suggest keeping added sugar well below 10% of daily calories — and lower is better. The biggest sources are sugary drinks, desserts and processed snacks, so those are the places to cut first.
