Sleep is active immune work

Sleep can look like doing nothing, but for your immune system it's prime working time. During deep sleep, the body produces and releases immune messengers called cytokines, immune cells become more effective, and the "memory" that lets your body recognise past threats is strengthened. Skimp on sleep and you blunt all of this — leaving your defences working at half power.

What is happening while you are asleep

The detail behind that is worth knowing, because it explains why the effect is not simply about feeling rested. Your body runs on a daily rhythm, and immune activity is part of it: the hormonal balance that prevails during the night differs from the daytime one, with the stress hormones that suppress immune activity at their lowest and the restorative signals of deep sleep at their highest. The evidence is striking

In a study where healthy volunteers were exposed to a cold virus after their sleep had been tracked, those who slept less were substantially more likely to develop a cold than those who slept seven hours or more (Prather et al., 2015).

A two-way relationship

The connection runs both ways.

Reviews describe a bidirectional relationship between sleep and immunity: adequate sleep supports host defence against infection, while sleep deprivation impairs immune parameters and promotes a chronic inflammatory state (bidirectional sleep-immunity review, 2015).

Poor sleep also weakens the antibody response to vaccines and reduces natural killer cell activity — the cells that patrol for virus-infected cells. In short, sleep isn't a luxury for your immune system; it's infrastructure.

Whose sleep is most at risk

That loop is why studies of sleep and immune system function pay close attention to people whose nights are disrupted as a matter of routine rather than by choice. Shift workers, and night workers in particular, are asking their bodies to sleep against the daily rhythm described above. New parents and people caring for someone unwell lose sleep in fragments, which is not the same as simply going to bed late. Insomnia, chronic pain, untreated sleep apnoea and anxiety all break sleep up from the inside, so the hours in bed overstate the sleep actually obtained. How sleep affects health beyond immunity

Immunity is only one thread. Blood pressure normally dips overnight, and sleep that is too short or too disrupted blunts that dip. What this doesn't mean

A few caveats keep this honest. One rough night is not a health event, and the research concerns habitual short sleep rather than the occasional late one. Nor does the link run in only one direction: illness, pain and stress all disturb sleep in their own right, so an association between poor nights and falling ill does not by itself settle which came first. The experiments that restrict sleep deliberately tend to be small and short, run in laboratories with healthy volunteers, which is a good way to observe a mechanism and a poor way to judge how much it adds up to over years. Good sleep is not an alternative to medical care either — it does not replace vaccination, treatment or advice about an infection, and no amount of it makes you immune to anything.
